    Acceptance of Terms

    By booking an appointment or using our services, you agree to these terms and conditions.

    Service Description

    Eye See You Optometry provides on-site optometry services to aged care facilities. Our services include comprehensive eye examinations, prescription services, and spectacle fitting.

    Appointment Terms

    Booking and Scheduling

    • Appointments must be booked in advance through our booking system
    • We require at least 48 hours notice for appointment confirmation
    • Appointments are subject to availability and practitioner schedules
    • We reserve the right to reschedule appointments due to unforeseen circumstances

    Cancellation Policy

    • Cancellations must be made at least 24 hours in advance
    • Late cancellations may incur a service fee
    • Emergency cancellations will be considered on a case-by-case basis
    • Repeated no-shows may result in booking restrictions
    Payment Terms

    Medicare and Insurance

    • We bulk-bill Medicare for eligible eye examinations
    • Private health insurance may cover additional services
    • Patients are responsible for verifying their coverage
    • Co-payments may apply for certain services

    Spectacle Services

    • Payment is required upon delivery of spectacles
    • Payment plans may be available on a case-by-case basis
    • Warranty terms apply to all spectacle orders
    • Refunds are subject to our refund policy
    Professional Standards

    Qualified Practitioners

    • All optometrists are registered with AHPRA
    • Services comply with Australian optometry standards
    • Continuing professional development is maintained
    • Professional indemnity insurance is current

    Scope of Practice

    • Services are limited to optometric scope of practice
    • Referrals to ophthalmologists may be required
    • Emergency eye conditions require immediate medical attention
    • Second opinions are welcome and encouraged
    Patient and Facility Responsibilities

    Information Sharing

    • Facility staff to provide available medical history and current information
    • Inform us of any changes to patient contact details or care arrangements
    • Notify us of any medication changes where possible
    • Report any adverse reactions or concerns on behalf of patients

    Cooperation and Care Environment

    • Assist patients to follow examination instructions and procedures as able
    • Ensure patients are available for scheduled appointments
    • Maintain a respectful environment for all residents and staff
    • Maintain confidentiality of all residents' health information
    Liability and Disclaimers

    Service Limitations

    • Services are provided in good faith and with reasonable skill
    • Results may vary based on individual circumstances
    • No guarantee of specific outcomes can be made
    • Professional judgment may override patient preferences

    Limitation of Liability

    Our liability is limited to the extent permitted by law. We maintain professional indemnity insurance as required by professional standards. Any claims must be made within the statutory limitation period.
